EXPLICIT CONSENT DECLARATION REGARDING THE PROCESSING, USE AND SHARING OF MY PERSONAL DATA BY DR. FAT\u0130H S\u00d6YLEMEZ<\/h2>\n

I hereby acknowledge, declare, and undertake that I have been verbally and in writing informed and enlightened, that I have read and reviewed the Clarification Text on the date of signature, and that I have fully understood and accepted all matters stated therein.
With my free will, I give explicit consent<\/strong> to the processing, use, and transfer of my personal data<\/strong> within the scope defined under the Law No. 6698 on the Protection of Personal Data (PDPL)<\/strong>.
This Consent Declaration<\/strong> shall remain valid unless I explicitly revoke it in writing.<\/p>\n

I hereby expressly consent<\/strong> to the collection, use, disclosure, transfer, and processing of any information I have shared with DR. FAT\u0130H S\u00d6YLEMEZ<\/strong> that constitutes personal data or special categories of personal data, in connection with or arising from any relationship I have established with DR. FAT\u0130H S\u00d6YLEMEZ<\/strong>, for the purpose of fulfilling the obligations imposed by the Ministry of Health<\/strong> or other administrative or judicial authorities related to such relationship, or for statistical and scientific research purposes<\/strong>, without prejudice to my rights under the applicable legislation.<\/p>\n

I further acknowledge, declare, and undertake that I shall obtain, protect, process, and transfer any personal data I may acquire due to my relationship with DR. FAT\u0130H S\u00d6YLEMEZ<\/strong> in accordance with the Law No. 6698 on the Protection of Personal Data<\/strong>, based on explicit consent and in full compliance with the law;
that I shall immediately delete any personal data obtained without explicit consent or in violation of the law and notify the situation;
and that in case of detection of any such unlawful act, if any sanctions are imposed by the Personal Data Protection Board<\/strong> or other administrative or judicial authorities, or if DR. FAT\u0130H S\u00d6YLEMEZ<\/strong> suffers any damage, I shall compensate for such damage, and I understand that I shall bear any criminal liability<\/strong> arising therefrom.<\/p>\n
