EXPLICIT CONSENT DECLARATION REGARDING THE PROCESSING, USE AND SHARING OF MY PERSONAL DATA BY DR. FATİH SÖYLEMEZ
I hereby acknowledge, declare, and undertake that I have been verbally and in writing informed and enlightened, that I have read and reviewed the Clarification Text on the date of signature, and that I have fully understood and accepted all matters stated therein.
With my free will, I give explicit consent to the processing, use, and transfer of my personal data within the scope defined under the Law No. 6698 on the Protection of Personal Data (PDPL).
This Consent Declaration shall remain valid unless I explicitly revoke it in writing.
I hereby expressly consent to the collection, use, disclosure, transfer, and processing of any information I have shared with DR. FATİH SÖYLEMEZ that constitutes personal data or special categories of personal data, in connection with or arising from any relationship I have established with DR. FATİH SÖYLEMEZ, for the purpose of fulfilling the obligations imposed by the Ministry of Health or other administrative or judicial authorities related to such relationship, or for statistical and scientific research purposes, without prejudice to my rights under the applicable legislation.
I further acknowledge, declare, and undertake that I shall obtain, protect, process, and transfer any personal data I may acquire due to my relationship with DR. FATİH SÖYLEMEZ in accordance with the Law No. 6698 on the Protection of Personal Data, based on explicit consent and in full compliance with the law;
that I shall immediately delete any personal data obtained without explicit consent or in violation of the law and notify the situation;
and that in case of detection of any such unlawful act, if any sanctions are imposed by the Personal Data Protection Board or other administrative or judicial authorities, or if DR. FATİH SÖYLEMEZ suffers any damage, I shall compensate for such damage, and I understand that I shall bear any criminal liability arising therefrom.
